What it is, How it works

Hair testing is viewed as an effective method for identifying drug and alcohol consumption. Hair acts as a repository for long-term alcohol and drug history by embedding biomarkers within the growing hair strand's fibers. When hair is cut nearest to the scalp, it can offer around a 3-month period for detecting alcohol and drugs. It's easy to collect, somewhat hard to tamper with, and simple to send.

A 1.5-inch sample consisting of approximately 200 hair strands (comparable to the width of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal sample size for screening and validation. For EtG, additional tests, and/or those beyond 10 panels, 150mg of the sample is advised. We suggest measuring the sample using a jeweler’s scale. Should scalp hair be unavailable, an equivalent quantity of body hair can be collected. References to head hair pertain only to scalp hair. Body hair encompasses other types such as facial, axillary, etc.

Process Overview

The key steps in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary processing of a sample into a lab's system. This step verifies the sample's proper sealing and shipment, assigns a random LAN (Laboratory Accessioning Number), and finalizes any extra data entry not incorporated by an electronic chain of custody system.

Screening offers an initial rapid assessment for drug abuse. Although Screening effectively eliminates non-usage in most samples, a positive result requires confirmation to be legally binding. Any samples preliminarily positive during Screening necessitate further confirmation.

If a sample is initially positive at the Screening stage, additional hair is extracted from the initial sample and prepped for Extraction. During Extraction, drugs are isolated from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening the most challenging to execute.

Confirming any positive Screening outcome is done via GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all tabled positive samples undergo necessary cleansing. The complete analytic process from Accessioning to Confirmation is subjected to review under the CAP (College of American Pathologists) Hair designation and complies with ISO / IEC 17025 standards.
