What it is, How it works

When collected near the scalp, hair can offer a detection window of up to approximately 90 days for alcohol and drugs. Hair is uncomplicated to gather, relatively resistant to tampering, and straightforward to transport.

A 1.5-inch sample consisting of about 200 hair strands (roughly the thickness of a #2 pencil) nearest to the scalp provides 100mg of hair, which is the optimal sample for both screening and validation. For EtG, additional tests, and/or panels exceeding 10, 150mg of the sample is advisable. We suggest using a jeweler’s scale to measure the sample weight. If scalp hair is not present, an equivalent quantity of body hair can be used. When mentioning head hair, we mean scalp hair specifically. Body hair pertains to all other kinds of hair (facial, axillary, etc.).

Procedure Summary

The laboratory processing of a drug test result includes four key actions: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the process of initiating a sample into a laboratory’s framework. This step involves ensuring the sample was properly sealed and shipped, generating a random LAN (Laboratory Accessioning Number), and finalizing any additional data entry not captured by an electronic chain of custody system.

Screening is an initial rapid assessment for drug abuse. While Screening is an economical method to exclude drug use in most samples, a positive outcome requires affirmation to be court-admissible. Any samples that indicate a presumptive positive in Screening necessitate a follow-up confirmation.

If a sample indicates a presumptive positive during the Screening phase, additional hair is extracted from the original sample for Extraction. At this juncture, drugs are isolated from hair at much lower levels than in other methods (e.g., urine or oral fluid), which renders hair drug screening the most complex method to execute.

Any positive screening findings undergo Confirmation through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are cleansed before confirmation as needed. The entire laboratory process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair categorization and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
