A 1.5-inch sample comprising around 200 hair strands (approximately the size of a #2 pencil) closest to the scalp yields 100mg of hair, which is the ideal sample size for analysis and verification. For EtG, supplementary tests, and/or tests exceeding 10 panels, 150mg of the sample is advised. It's recommended to weigh the specimen using a jeweler’s scale. If scalp hair is not an option, a similar quantity of body hair can be used. When mentioning head hair, we specifically mean scalp hair. Body hair refers to any other hair types (like facial or axillary hair).

Process Overview

The four primary stages in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the preliminary processing of a specimen into a laboratory’s system. This involves confirming that the sample was correctly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry that an electronic chain of custody system has not provided.

Screening is a preliminary rapid assessment for drugs of abuse. While Screening is a cost-effective method to rule out drug use on the majority of samples, a positive screen must be confirmed to be legally admissible. Samples that are presumptively positive in Screening necessitate a secondary confirmation.

If a specimen is presumptively positive at the Screening phase, further hair is extracted from the initial sample and prepared for Extraction. In this phase, drugs are extracted from hair at much lower concentrations than in other methods (e.g., urine or oral fluid), which is why hair drug screening is the most challenging to execute.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before confirmation as required. The complete lab process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
