What it is, How it works

When taken near the scalp, it offers nearly a 3-month detection period for both alcohol and drugs. Hair is easy to gather, challenging to tamper with, and convenient to transport.

A 1.5-inch portion comprising around 200 strands of hair (nearly the width of a #2 pencil) nearest to the scalp provides 100mg of hair, which is the optimal quantity for both evaluation and verification. For EtG, supplementary, and/or tests exceeding 10 panels, 150mg of hair is advisable. We suggest measuring the specimen's weight using a jeweler’s scale. If scalp hair is not available, an equivalent quantity of body hair is acceptable. When discussing head hair, it refers exclusively to scalp hair. Body hair pertains to all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial integration of a sample into a lab’s system. This process checks if the sample was appropriately sealed and dispatched, assigns a random LAN (Laboratory Accessioning Number), and completes any further data entry beyond what an electronic chain of custody system provides.

Screening is a preliminary rapid assessment for drugs of abuse. Although Screening is an economical method to rule out drug use in most samples, any positive indication must be confirmed for court admissibility. Any samples that are initially positive in Screening need follow-up confirmation.

When a sample shows a preliminary positive in the Screening stage, more hair is drawn from the set sample and prepared for Extraction. This stage involves drug extraction from hair in much lower concentrations compared to other techniques (e.g., urine or oral fluid), rendering hair drug screening the most challenging method.

Confirmation of any positive screening result is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation if necessary. The complete lab routine from Accessioning to Confirmation is overseen under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
