When extracted near the scalp, hair can offer roughly a 3-month detection range for both alcohol and drugs. Hair is straightforward to collect, somewhat challenging to tamper with, and convenient for transport.

A 1.5-inch hair sample consisting of around 200 strands (roughly the size of a #2 pencil) near the scalp amounts to 100mg, which is the optimal sample size for both screening and confirmation. For EtG, add-ons, and/or analyses exceeding 10 panels, 150mg of the sample is suggested. We advocate weighing the hair sample via a jeweler’s scale. If scalp hair cannot be sourced, an equivalent volume of body hair may be gathered. Note that head hair specifically refers to scalp hair. Body hair denotes all other hair types (facial, axillary, etc.).

Process Overview

The processing of a drug test outcome in the lab encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary integration of a sample into a laboratory's database. This phase covers the verification that the sample was appropriately sealed and dispatched, allocating a random LAN (Laboratory Accessioning Number), and finalizing any additional data processing not managed by an electronic chain of custody mechanism.

Screening signifies an initial rapid assessment for substance abuse. Though Screening is an economical approach to eliminating drug consumption in most samples, a positive screening result must be validated for legal admissibility. All samples yielding a presumptive positive during Screening require further confirmation.

This stage involves retrieving drugs from hair at significantly reduced concentrations compared to other techniques (such as urine or oral fluid), marking hair drug screening as the most challenging method to execute.

Confirmation for any positive screening result is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive specimens undergo a wash before confirmation as necessary. The full laboratory sequence from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.
