When collected near the scalp, hair can offer up to nearly a 3-month detection period for alcohol and other substances. It’s straightforward to collect, relatively hard to tamper with, and straightforward to transport.

A 1.5-inch sample of around 200 hair strands (approximately the size of a #2 pencil) closest to the scalp will yield 100mg of hair, the optimal sample for screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the specimen is advised. It's suggested to measure the specimen on a jeweler’s scale. If scalp hair is not available, a comparable amount of body hair might be gathered. When referencing head hair, we mean scalp hair solely. Body hair pertains to all other hair categories (facial, axillary, etc.).

Process Overview

The primary four steps involved in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the preliminary processing of a sample into a lab’s system. This phase includes verifying that the sample was correctly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry absent from an electronic chain of custody system.

Screening involves an initial fast check for abusive drug use. Though Screening is a cost-effective means to rule out drug usage on most samples, a positive screen requires confirmation to be court-admissible. Any samples that are presumptively positive during Screening do require subsequent confirmation.

If a sample is suspected positive during the Screening phase, additional hair is acquired from the primary specimen and prepared for Extraction. During this stage, drugs are extracted from hair at a much lower level than in other methods (ex. urine or oral fluid), making hair drug screening the toughest method to conduct.

Confirmation of any presumptive positive screening result is carried out via GC/MS, GC/MS/MS or LC/MS/MS. All suspected positive samples are cleansed before confirmation as required. The full lab process from Accessioning to Confirmation is inspected under both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
