For EtG, additional testing, or assessments exceeding 10 panels, 150mg of the sample is advised. Using a jeweler’s scale for weighing the sample is recommended. If hair from the scalp isn't available, an equivalent quantity of body hair can be used. References to head hair are specific to scalp hair only, whereas body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The primary stages in the laboratory handling of a drug test result consist of Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial integration of a sample into the lab’s system, which includes confirming proper sealing and shipping of the sample, assigning a unique LAN (Laboratory Accessioning Number), and completing any additional data entry omitted in an electronic chain of custody system.

Screening entails an initial swift examination for drug traces. Although Screening offers a budget-friendly means to exclude drug presence in most samples, a positive outcome requires confirmation to hold up in legal contexts. Samples with a presumptive positive result during Screening necessitate a further confirmation process.

In cases where a sample yields a presumptive positive in the Screening phase, further hair is retrieved from the original sample and readied for Extraction. During this process, drugs are isolated from hair at concentrations much lower than other methods (e.g., urine or oral fluid), attributing to the complexity of performing hair drug screening.

Confirmation of any positive screening result is performed through GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation as required. The whole laboratory process, from Accessioning through Confirmation, is evaluated under both the CAP (College of American Pathologists) Hair accreditation and the ISO / IEC 17025 standards.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
