What it is, How it works

Hair testing is increasingly recognized as a valuable method for identifying drug and alcohol consumption. Hair retains a long-term record of these substances by capturing biomarkers within the fibers of the hair as it grows. When collected from near the scalp, hair can offer a detection window of up to three months for alcohol and drugs. Hair is easy to collect, challenging to tamper with, and convenient to transport.

A sample measuring 1.5 inches, consisting of about 200 strands (roughly the thickness of a #2 pencil) taken closest to the scalp, equates to 100mg, the optimal amount for both screening and confirmation testing. For EtG, additional tests, or tests exceeding 10 panels, 150mg is advisable. It's recommended to weigh the sample using a jeweler’s scale. If scalp hair isn't available, the equivalent amount of body hair can be collected. References to head hair specifically mean scalp hair, whereas body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the sample's initial processing into the laboratory's system. This step includes confirming the sample's proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not provided by an electronic chain of custody system.

Screening involves an initial rapid screening for drugs of abuse. While this method is cost-effective for ruling out drug use in most samples, positive screens must be confirmed to be legally admissible. Samples that are presumptively positive during Screening require secondary confirmation.

For presumptive positive results in Screening, additional hair is extracted from the initial sample for further Extraction. This stage involves extracting drugs from hair at much lower concentrations compared to other testing methodologies (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to perform.

Positive screening results undergo Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. Any samples with a presumptive positive result are washed before confirmation as necessary. The entire process, from Accessioning to Confirmation, is conducted under both the CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 accreditation standards.

The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. Furthermore, the SoHT meeting in Verona featured research on the identification and quantification of alcohol abuse biomarkers, their stability, and their correlation with exposure and other biomarkers.

Forensic Applications and Interpretation: One of the vital objectives of hair testing is its application in forensic investigations and legal proceedings. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
