What it is, How it works

Hair offers a prolonged history of alcohol and substances by capturing markers within the fibers of the developing hair strand. When taken near the scalp, hair can offer roughly a 3-month detection period for alcohol and substances. Hair is easy to gather, somewhat challenging to alter, and simple to dispatch.

A 1.5-inch segment consisting of roughly 200 hair strands (about the diameter of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal sample for examination and validation. For EtG, additional tests, and/or assessments exceeding 10 panels, 150mg of the sample is advised. Utilizing a jeweler’s scale for weighing the sample is recommended. If scalp hair is not available, a comparable quantity of body hair can be gathered. When mentioning head hair, we specifically mean scalp hair. Body hair includes all other hair forms (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial setup of a sample into the laboratory’s system. This process verifies the sample was correctly sealed and dispatched, allocates a random LAN (Laboratory Accessioning Number), and finalizes any additional data entry missing from an electronic chain of custody setup.

Screening offers a preliminary assessment for substance misuse. Though Screening provides a cost-effective means to exclude drug consumption in most samples, a positive screen requires confirmation for use in legal proceedings. Samples presumed positive in Screening require follow-up confirmation.

If a sample is projected as positive during the Screening phase, additional hair is collected from the original sample for Extraction. In this phase, substances are drawn from hair at substantially lower concentrations than other procedures (e.g., urine or saliva), representing a challenge in hair drug evaluation.

Verification of any positive screening outcome is completed with GC/MS, GC/MS/MS or LC/MS/MS. Any presumptive positive samples are washed if necessary before verification. The entire laboratory sequence from Accessioning to Confirmation is reviewed per the CAP (College of American Pathologists) Hair criteria and conforms to ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
