A sample measuring 1.5 inches from approximately 200 strands (about the width of a #2 pencil) nearest to the scalp yields 100mg, the recommended amount for screening and confirmation. For EtG, extended panels, or tests beyond 10 panels, 150mg is advisable. We suggest using a jeweler’s scale for accurate measurement. If scalp hair is unavailable, an equal quantity of body hair may be gathered. Referring to head hair specifies scalp hair only, while body hair encompasses all other types (facial, axillary, etc.).

Process Overview

Laboratory processing of a drug test result includes four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a lab’s system. This involves ensuring proper sealing and shipping, assigning a random Laboratory Accessioning Number (LAN), and completing any necessary data entry not covered by an electronic chain of custody.

Screening serves as a preliminary assessment for drugs of abuse. While Screening offers a cost-effective approach to rule out drug use for most samples, a positive result requires confirmation to be legally admissible. Any samples that test positive in Screening necessitate a further confirmation step.

Should a sample yield a presumptive positive during Screening, more hair is extracted from the original sample and prepared for Extraction. This stage involves extracting drugs from hair at a significantly lower concentration compared to other methods (e.g., urine or oral fluid), making hair drug screening a challenging methodology.

Confirmation of any positive screen result employs techniques such as GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are cleansed before confirmation if necessary. The complete laboratory process from Accessioning to Confirmation is reviewed under both CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
