For EtG, additional tests, and/or those surpassing 10 panels, it’s suggested to use 150mg of the sample. It's advisable to weigh the hair sample using a jeweler’s scale. If scalp hair is not an option, an equivalent quantity of body hair can be used. When the term head hair is used, it specifically indicates scalp hair. Body hair pertains to other varieties (facial, underarm, etc.).

Process Overview

There are four crucial stages in processing drug test results in the lab: Accessioning, Screening, Extraction, and Confirmation.

In Accessioning, a sample is initially processed into the laboratory's system. This process ensures the sample is properly sealed and dispatched, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data input not covered by an electronic chain of custody system.

Screening provides a quick preliminary test for substance abuse. Although Screening is a cost-efficient method to rule out drug use for most samples, following up with confirmation is essential for legal admissibility. Any samples that presumptively test positive during Screening must undergo a secondary confirmation.

Upon a presumptive positive result in Screening, more hair is extracted from the primary sample for the Extraction stage. Here, drugs are extracted from hair at a much lower concentration compared to other methodologies (such as urine or oral fluid), highlighting the complexity of performing hair drug screening.

GC/MS, GC/MS/MS, or LC/MS/MS methods handle the Confirmation of any positive screening outcomes. Prior to confirmation, any samples with presumptive positive results are washed as necessary. The entire process, from Accessioning to Confirmation, is scrutinized under the CAP (College of American Pathologists) Hair standards and accredited to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
