For tests that include EtG, add-ons, and those extending beyond 10 panels, it is advisable to have 150mg of the specimen. Using a jeweler’s scale to ascertain the specimen’s weight is suggested. If scalp hair cannot be obtained, the equivalent amount of body hair may be an alternative. The term 'head hair' is solely applicable to scalp hair, while 'body hair' pertains to all other hair categories (such as facial, axillary, etc.).

Process Overview

The drug test result processing in a laboratory is compartmentalized into four predominant phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning denotes the preliminary integration of a sample into a lab's system. This step encompasses validating that the sample was correctly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not accommodated by an electronic chain of custody system.

Screening serves as an initial swift assessment for drugs of abuse. Despite its cost-effectiveness in ruling out drug use in most samples, screening results need affirmation to be recognized in legal settings. Every specimen that shows presumptive positivity in Screening requires an ensuing confirmation.

For samples flagged as presumptively positive during Screening, extra hair is sourced from the primary specimen for the Extraction process. At this stage, drugs are isolated from hair at significantly lower concentrations compared to other procedures (e.g., urine or oral fluid), which makes hair drug screening one of the most challenging methodologies.

Confirmation of any positive initial screening result is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are pre-cleaned before confirmation as necessary. The entire lab process, spanning from Accessioning to Confirmation, undergoes review under the CAP (College of American Pathologists) Hair criteria along with accreditation aligned with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
