Hair testing is widely regarded as a potent method for detecting the use of drugs and alcohol. By capturing biomarkers within the fibers of the hair that is growing, hair provides a historical overview of alcohol and other drugs. When taken near the scalp, hair may offer up to an approximate 3-month detection period for various drugs. Hair collection is straightforward, relatively resistant to tampering, and easy to transport.
Taking a 1.5-inch segment of about 200 hair strands (similar in size to a #2 pencil) nearest to the scalp provides 100mg of hair, which is ideal for screening and confirmation. For EtG, additional assessments, and/or tests exceeding 10 panels, 150mg of the hair sample is advised. We advocate using a jeweler's scale to weigh the specimen. If scalp hair is not available, an equivalent sample of body hair can be used. By head hair, we mean scalp hair only, while body hair includes all other types (facial, axillary, etc.).
Process Overview
There are four principal stages in processing a drug test result in a laboratory: Accessioning, Screening, Extraction, and Confirmation.
Accessioning includes the preliminary sample processing in a laboratory system, ensuring proper sealing and shipping of the sample, assigning an arbitrary LAN (Laboratory Accessioning Number), and completing any additional data input absent from an electronic chain of custody system.
Screening consists of a preliminary rapid assessment for illicit drugs. While Screening offers a cost-effective approach to exclude drug use in most samples, any positive Screen result must be confirmed for legal admissibility. Samples deemed presumptively positive in Screening necessitate further confirmation.
When a sample is found to be presumptively positive during Screening, more hair is retrieved from the initial sample for preparation during Extraction. During this phase, drugs are extracted from the hair at significantly reduced concentrations compared to other methods (e.g., urine or oral fluid), demonstrating why hair drug screening is the most challenging method to execute.
Confirmation is conducted for any positive screen result through the use of GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing as needed before Confirmation. The complete laboratory process from Accessioning to Confirmation undergoes review in compliance with both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.
