If scalp hair is not available, an equivalent sample of body hair can be used. By head hair, we mean scalp hair only, while body hair includes all other types (facial, axillary, etc.).

Process Overview

There are four principal stages in processing a drug test result in a laboratory: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary sample processing in a laboratory system, ensuring proper sealing and shipping of the sample, assigning an arbitrary LAN (Laboratory Accessioning Number), and completing any additional data input absent from an electronic chain of custody system.

Screening consists of a preliminary rapid assessment for illicit drugs. While Screening offers a cost-effective approach to exclude drug use in most samples, any positive Screen result must be confirmed for legal admissibility. Samples deemed presumptively positive in Screening necessitate further confirmation.

When a sample is found to be presumptively positive during Screening, more hair is retrieved from the initial sample for preparation during Extraction. During this phase, drugs are extracted from the hair at significantly reduced concentrations compared to other methods (e.g., urine or oral fluid), demonstrating why hair drug screening is the most challenging method to execute.

Confirmation is conducted for any positive screen result through the use of GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing as needed before Confirmation. The complete laboratory process from Accessioning to Confirmation undergoes review in compliance with both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
