Hair analysis has become recognized as an effective method for identifying drug and alcohol consumption. Hair offers a comprehensive timeline of substance use by encapsulating biomarkers within the strands as they grow. When taken from the scalp, hair testing can reveal up to an estimated 90-day history of alcohol and drug use. Collecting hair is straightforward, moderately challenging to tamper with, and convenient to transport.
An optimal testing sample includes a 1.5-inch length of approximately 200 hair strands (comparable to the width of a pencil), yielding 100mg of hair, suitable for both screening and confirmation tests. For EtG, additional tests, and/or panels exceeding 10, 150mg is preferred. It’s advisable to measure the sample with a jeweler’s scale. If scalp hair is not available, a similar quantity of body hair could be used. In this context, head hair refers specifically to scalp hair, while body hair includes all other forms (facial, axillary, etc.).
Process Overview
The drug testing procedure in the laboratory comprises four key phases: Accessioning, Screening, Extraction, and Confirmation.
Accessioning deals with the preliminary handling of a sample in a lab’s framework. This process ensures the sample is sealed and transported correctly, assigns it a unique LAN (Laboratory Accessioning Number), and completes necessary data entry absent in an electronic chain of custody setup.
Screening is a rapid assessment for detecting abuse of drugs. Although cost-efficient to rule out drug presence in most samples, any positive screen must be verified for legal suitability. Samples showing positive results in Screening necessitate further confirmation.
In cases where Screening indicates a positive result, additional hair is extracted from the initial sample and processed for Extraction. During this phase, drugs are isolated from hair at a much lower concentration compared to other methods (e.g., urine or oral fluid), which makes hair drug testing particularly challenging to execute.
Any positive Screening results undergo Confirmation through GC/MS, GC/MS/MS or LC/MS/MS. Prior to Confirmation, all positive samples are washed if required. From Accessioning to Confirmation, the entire laboratory workflow is scrutinized under both the CAP (College of American Pathologists) Hair guidelines and ISO / IEC 17025 accreditation standards.
