For EtG, additional tests, and/or tests beyond 10 panels, it's recommended to collect 150mg of the specimen. We suggest weighing the sample using a jeweler’s scale. If scalp hair is not available, an equivalent amount of body hair can be used. When we mention head hair, it is specifically referencing scalp hair, while body hair includes all other types (such as facial and axillary hair).

Process Overview

The laboratory processing of a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage where a sample is entered into a laboratory’s system. This step includes ensuring the sample was properly sealed and transported, generating a random LAN (Laboratory Accessioning Number), and completing any additional data entry if not provided by an electronic chain of custody system.

Screening is a preliminary check for drugs of abuse. While it is a cost-effective approach to rule out drug use in most samples, any positive result from Screening must be confirmed to be usable in court. All samples that test presumptively positive in Screening require a secondary confirmation process.

When a sample tests positive in the Screening phase, additional hair is extracted from the original specimen and prepared for Extraction. In this stage, drugs are extracted from the hair at a significantly lower concentration compared to other methods (such as urine or oral fluid). This makes hair drug screening the most challenging methodology to perform.

Confirmation of positive screening results is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before confirmation as necessary. The complete laboratory process from Accessioning to Confirmation is reviewed under the CAP (College of American Pathologists) Hair designation as well as accreditation to ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
