What it is, How it works

Hair examinations are increasingly recognized as an effective means for identifying substance and alcohol consumption. The hair serves as a repository of long-term substance markers, capturing evidence within the fibers as the hair grows. Hair samples taken near the scalp can provide detection insights for up to three months for alcohol and illicit substances. Collecting hair is straightforward, challenging to tamper with, and convenient for transportation.

A 1.5-inch segment comprising around 200 hair strands (approximately the diameter of a #2 pencil) when taken close to the scalp will yield 100mg, which is ideal for both screening and confirmation. For EtG, additional tests, and panels exceeding 10, it's advisable to use 150mg of the sample. Utilizing a jeweler’s scale is recommended for precise measurement. In the absence of scalp hair, equivalent amounts of body hair can be used. When discussing head hair, it specifically refers to scalp hair, whereas body hair encompasses all other categories (facial, axillary, etc.).

Process Overview

There are four critical phases in the analytical processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary entry of a specimen into the laboratory's system. This process includes checking the sample's sealing and shipping integrity, assigning a unique LAN (Laboratory Accessioning Number), and completing any additional data entry requirements not covered by an electronic chain of custody protocol.

Screening involves an initial cursory analysis for the presence of substances. While Screening is a cost-efficient method for eliminating drug use in most specimens, any positive findings must undergo confirmation to ensure they hold up in legal proceedings. Specimens that yield initial positive results require further verification.

Should a specimen exhibit presumptive positivity during the Screening phase, additional hair is extracted from the initial sample and subjected to Extraction. During this step, substances are extracted from the hair in much lesser concentrations compared to other methods (e.g., urine or oral fluids), rendering hair drug screening a challenging practice to execute.

Confirmation of any positive findings from Screening is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed prior to confirmation as needed. The entire process from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation and accredited to ISO/IEC 17025 standards.

Benefits of hair drug analysis include:

  • Extended detection range: Hair drug tests can reveal substance use for up to 90 days unlike urine tests with shorter detection spans.
  • Challenging to falsify results: Hair drug tests are tough to tamper with, ensuring result reliability.
  • Tracks historical usage: These tests can highlight usage patterns over time instead of only recent usage.

Considerations:

  • Inability to detect recent usage: Drugs typically become detectable in hair within 5-7 days.
  • Expense: Hair drug analyses are generally pricier than alternative drug testing methods.
  • Result variability: Elements such as hair color and individual growth rates can influence metabolite concentrations in hair.

Notice: Despite being commonly termed as "hair follicle tests," the analysis evaluates the hair strand itself and not the follicle beneath the scalp.
