What it is, How it works

Hair testing is now recognized as an effective means for identifying drug and alcohol consumption. Hair offers a prolonged timeline of alcohol and drug usage by embedding biomarkers within the strands of growing hair. When taken from close to the scalp, the hair can reveal a detection window of up to around 3 months for alcohol and drugs. The collection process for hair is straightforward, challenging to falsify, and simple to transport.

A sample measuring 1.5 inches consisting of roughly 200 hair strands (approximately the thickness of a #2 pencil) nearest the scalp will yield 100mg of hair, which is the perfect sample size for testing and confirmation. For EtG, additional screenings, and/or assessments beyond 10 panels, it is advised to use 150mg of the sample. We suggest using a jeweler’s scale to weigh the sample. Should scalp hair be unavailable, a comparable quantity of body hair may be utilized. When discussing head hair, we specifically mean scalp hair. Body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

The principal stages in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into the lab’s system. This encompasses checks to ensure the sample was correctly sealed and delivered, generating a random LAN (Laboratory Accessioning Number), and performing any supplementary data entry not available via an electronic chain of custody system.

Screening comprises an initial rapid evaluation for substances of abuse. While Screening is an economical method to exclude drug usage in the majority of samples, a positive screening result must be confirmed to hold up in court. Samples that are determined to be presumptively positive in Screening necessitate additional confirmation.

Should a sample be presumptively positive during the Screening phase, extra hair is extracted from the initial sample, prepared for Extraction. At this stage, drugs are withdrawn from hair at significantly reduced concentrations compared to other techniques (e.g., urine or oral fluid), thus rendering hair drug screening one of the most challenging methodologies to execute.

Confirmation of any positive screening outcome is accomplished using GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples undergo a washing prior to confirmation if necessary. The full laboratory process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and the certification to ISO / IEC 17025 standards.

Advantages of hair drug testing:

• Challenging to circumvent: It is extremely challenging to deceive a hair drug test, enhancing the credibility of the results.

With its multidisciplinary nature, hair testing brings together researchers, scientists, and experts from various domains to develop innovative techniques, explore new methodologies, and address emerging concerns, responding to the ever-evolving challenges of drug abuse, therapeutic drug monitoring, and environmental exposures to pollutants.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

Novel Analytical Techniques: The Verona meeting witnessed the unveiling of cutting-edge analytical techniques, offering enhanced sensitivity, selectivity, and accuracy for drug testing. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
