A sample of 1.5 inches in length consisting of approximately 200 strands of hair (roughly comparable to a #2 pencil in size) next to the scalp will yield 100mg of hair, the ideal quantity for screening and verification. For EtG, enhancements, and/or examinations exceeding 10 panels, it's advised to gather 150mg of the sample. We suggest measuring the sample using a jeweler’s scale. Should scalp hair be unavailable, an equal portion of body hair can be used. When discussing head hair, we specifically mean scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The primary stages in the lab handling of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the starting phase of processing a sample in a lab’s system. This process ensures the sample was properly sealed and transported, a unique LAN (Laboratory Accessioning Number) is assigned, and any additional information needed not included in an electronic chain of custody is completed.

Screening is a preliminary assessment for drugs of misuse. Though Screening efficiently and economically eliminates drug usage in most samples, any positive screen requires confirmation to be valid in court. Presumptive positive screenings necessitate an additional confirmation test.

If a sample shows presumptive positivity in the Screening phase, additional hair is retrieved from the initial sample and readied for Extraction. At this phase, drugs are extracted from hair but at significantly lower concentrations compared to other methods (such as urine or oral fluid), which makes hair drug testing an exceptionally challenging methodology.

Confirmation of a positive screening result is done using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that suggest positive results undergo washing before confirmation, as necessary. The complete lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair standards and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
