What it is, How it works

Hair testing is regarded as a highly effective method for identifying drug and alcohol consumption. The hair captures biomarkers in its fibers, providing a comprehensive history of substance use over an extended period. When obtained near the scalp, hair offers a detection window of up to three months for alcohol and drugs. Hair is straightforward to collect, challenging to tamper with, and convenient for shipping.

A 1.5-inch section consisting of approximately 200 hair strands (comparable to the width of a #2 pencil) nearest to the scalp yields 100mg of hair, the optimal sample size for both screening and confirmation. For EtG, additional tests, or when exceeding 10-panel tests, 150mg of the hair specimen is advised. It is recommended to weigh the sample using a jeweler’s scale. Should scalp hair be inaccessible, an equivalent amount of body hair can be collected. The term "head hair" refers specifically to scalp hair, whereas "body hair" pertains to all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning constitutes the initial processing phase, during which a sample is introduced into a laboratory’s system. This process involves verifying the sample's seal and shipment accuracy, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry that is not provided by an electronic chain of custody system.

Screening entails a preliminary assessment for drugs of abuse. Although Screening is a cost-efficient means to rule out drug consumption in most samples, a positive Screen requires confirmation to be court-admissible. Any samples returning a presumptive positive during Screening necessitate further confirmation.

If a sample shows a presumptive positive result during the Screening stage, additional hair is extracted from the initial specimen and prepared for Extraction. This phase involves extracting drugs from hair at significantly lower concentrations than other methodologies (e.g., urine or oral fluid), underscoring the challenges in conducting hair drug screening.

Any positive Screening result is confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation, as required. The entire laboratory operation from Accessioning to Confirmation is subject to review under both CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
