Reference to head hair pertains strictly to scalp hair, while body hair encompasses other kinds (such as facial or axillary).

Process Overview

Four primary stages are involved in laboratory evaluation of a drug test outcome: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary entry of a specimen into the lab's framework. Tasks include ensuring the sample's proper sealing and delivery, generating a random LAN (Laboratory Accessioning Number), and finalizing any supplementary data requirement absent from an electronic chain of custody protocol.

Screening is a preliminary swift check for drug presence. It's a budget-friendly approach to ascertain substance usage in the majority of cases, although a positive screening needs additional verification for legal validity. Any specimens flagged as presumptively positive at this stage require subsequent confirmation.

When a specimen is flagged as positive in the Screening phase, additional hair segments are taken from the initial sample and prepped for Extraction. This phase involves extracting drugs at lower concentrations when compared to other methods like urine or oral fluid analysis. Hair drug screening represents the most challenging technique to execute.

Confirmation of any positive outcomes from screening is pursued using GC/MS, GC/MS/MS, or LC/MS/MS. All suspected positive samples undergo cleansing before confirmation if necessary. The full laboratory procedure, from Accessioning through Confirmation, is assessed under the CAP (College of American Pathologists) Hair designation alongside certification to ISO / IEC 17025 criteria.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
