What it is, How it works

Hair testing is regarded as an effective method for identifying drug and alcohol usage. By capturing biomarkers within the fibers of the growing hair strand, hair provides an extended history of alcohol and other drug use. When sourced near the scalp, hair can offer up to roughly a 3-month detection period for alcohol and substance use. Hair is easy to collect, relatively hard to tamper with, and simple to send.

A 1.5-inch portion of around 200 hair strands (equivalent to the thickness of a #2 pencil) nearest the scalp will yield 100mg of hair, the perfect sample for analysis and verification. For EtG, additional parameters, and/or screenings beyond 10 panels, 150mg of the sample is advised. It is suggested to assess the sample using a jeweler’s scale. If scalp hair is not present, a similar portion of body hair can be used. By head hair, we specifically mean scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result comprises four major phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves integrating a sample into a lab’s system initially. This step includes confirming the sample was sealed and dispatched correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any lacking data entry not covered by an electronic chain of custody system.

Screening is a preliminary rapid test for controlled substances. Although Screening is an economical approach to rule out drug use for the majority of cases, a positive screen must be validated to stand up in court. Any samples that test positive in Screening necessitate additional confirmation.

If a sample tests positive in the Screening stage, more hair is extracted from the initial sample and prepared for Extraction. Drugs are extracted from hair at a significantly lower concentration compared to other methods (e.g., urine or oral fluid), making hair drug testing the most challenging methodology.

Confirmation for any positive screening result is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All prescreened positive samples are cleansed before confirmation as needed. The complete laboratory process from Accessioning to Confirmation is monitored under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
