What it is, How it works

Hair captures a longer-term record of alcohol and drugs by embedding biomarkers within the strands of hair as they grow. When taken from near the scalp, hair can offer around a 3-month window for detecting alcohol and drugs. Hair is straightforward to gather, somewhat challenging to tamper with, and simple to transport.

A 1.5-inch segment of roughly 200 hair strands (about the thickness of a #2 pencil) near the scalp will yield 100mg of hair, the optimal amount for both screening and verification. For EtG, additional tests, and/or panels exceeding 10, 150mg of the specimen is suggested. We advise measuring the specimen using a jeweler’s scale. If scalp hair is not available, an equal quantity of body hair may be gathered. When discussing head hair, we refer to scalp hair only. Body hair covers all other hair types (facial, axillary, etc.).

Process Overview

The key stages in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a lab’s system. This involves confirming the sample was properly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not included by an electronic chain of custody system.

Screening involves a preliminary quick assessment for drugs of abuse. Although Screening is a cost-effective method to dismiss drug use in most samples, a positive outcome requires confirmation for court admissibility. Any samples with a presumptive positive in Screening need a secondary confirmation.

If a sample shows a presumptive positive in the Screening phase, more hair is drawn from the initial sample and prepared for Extraction. In this phase, drugs are extracted from hair at significantly lower concentrations than in other methods (ex. urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Confirmation of any positive screening outcome is conducted via GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples undergo washing as needed prior to confirmation. The entire lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair specification and ISO / IEC 17025 accreditation standards.
