What it is, How it works

Hair testing has emerged as an effective method for identifying drug and alcohol consumption. Hair retains a long-lasting record of alcohol and drug intake by capturing biomarkers within the fibers as the hair grows. When gathered near the scalp, hair can reveal alcohol and drug use for up to about a 3-month period. It's easy to collect, relatively resistant to tampering, and simple to ship.

A 1.5-inch section of roughly 200 hair strands (about the size of a #2 pencil) nearest to the scalp ensures 100mg of hair, the ideal sample for both screening and confirmation. For EtG, add-ons, and/or testing beyond 10 panels, 150mg of hair is advised. It's suggested to weigh the sample using a jeweler’s scale. If scalp hair isn’t available, an equivalent amount of body hair can be used. When mentioning head hair, it refers exclusively to scalp hair. Body hair encompasses all other types, such as facial and axillary hair.

Process Overview

The laboratory process for drug testing consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary process of logging a sample into a lab's system. This phase checks that the sample is properly sealed and sent, generates a random LAN (Laboratory Accessioning Number), and completes any additional data entry not furnished by an electronic chain of custody.

Screening is a quick preliminary assessment for drugs of abuse. While it is a cost-efficient way to exclude drug use in most samples, a positive screen must be confirmed for legal admissibility. Samples preliminarily positive during Screening require a subsequent confirmation.

When a sample yields a presumptive positive in Screening, additional hair is prepared for Extraction. In this stage, drugs are extracted from hair at much lower concentrations than other methods (e.g., urine or oral fluid), making hair drug testing particularly challenging.

For a positive screening result, Confirmation is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are specially treated before Confirmation. The entire lab procedure from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
