For testing that includes EtG, supplements, or exceeds 10 panels, a 150mg specimen is advisable. Utilizing a jeweler's scale to measure the sample is suggested. Should scalp hair be inaccessible, the collection of an equivalent volume of body hair is acceptable. "Head hair" exclusively refers to hair on the scalp, whereas "body hair" pertains to all other hair forms (e.g., facial, underarm).

Process Overview

The laboratory protocol for processing a drug test result includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary handling of a sample into a lab's system. This step ensures the sample's seal and shipment reliability, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data entry absent from an electronic chain of custody system.

Screening conducts an initial rapid examination for drug traces. Although Screening is an economical means to dismiss drug usage across numerous samples, a positive result requires confirmation to hold up in legal settings. Samples with positive indications in Screening necessitate an additional confirmation.

Should a sample test positively during the Screening phase, additional hair is sourced from the initial sample and prepared for Extraction. This phase involves drug extraction from hair at considerably reduced concentrations compared to other techniques (such as urine or oral fluid), rendering hair drug monitoring among the most complex testing methods available.

Confirmation of any positively-screened results is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo necessary cleansing prior to confirmation. The entire laboratory procedure from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
