What it is, How it works

A sample measuring 1.5 inches containing roughly 200 hair strands (equivalent to the size of a #2 pencil) taken nearest to the scalp will yield 100mg of hair, suitable for screening and confirmation. For EtG, additional tests, and/or analyses exceeding 10 panels, 150mg of the sample is advised. It's suggested to weigh the sample using a jeweler's scale. If scalp hair is lacking, a similar amount of body hair may be used. Reference to head hair indicates scalp hair alone, whereas body hair includes all other kinds (facial, axillary, etc.).

Process Overview

The primary four stages of laboratory processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial process of integrating a sample into a lab's system. It ensures that the sample was sealed and transported correctly, allocates a random LAN (Laboratory Accessioning Number), and completes any extra data entry missing from an electronic chain of custody process.

Screening provides an initial rapid assessment for drug presence. While it's an economical manner to exclude drug use for most samples, a positive result in Screening must be verified for courtroom acceptability. Samples posing a preliminary positive result during Screening require further confirmation.

When a sample shows a preliminary positive in Screening, additional hair is extracted from the initial sample and prepped for Extraction. In this phase, drugs are extracted from hair at significantly reduced concentrations than those in other methods (like urine or oral fluid), which makes hair drug screening the most complex method to undertake.

Confirmation of a positive screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. Each preliminary positive sample undergoes washing before confirmation if needed. The lab process from Accessioning to Confirmation undergoes review under both CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 accreditation standards.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
