When obtained near the scalp, hair can reveal up to a roughly 3-month period of detection for alcohol and substances. Hair is easy to gather, challenging to tamper with, and simple to transport.

A 1.5-inch section containing around 200 hairs (about the size of a #2 pencil) nearest to the scalp will provide 100mg of hair, the optimal quantity for testing and validation. For EtG, additional screenings, or tests surpassing 10 panels, 150mg of the sample is advised. We advise using a jeweler's scale for measuring the sample. If scalp hair is unavailable, a similar quantity of body hair can be used. References to head hair mean scalp hair only. Body hair pertains to all other types (facial, axillary, etc.).

Process Overview

The essential steps in the lab handling of drug test results are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the sample's preliminary processing into a lab's system. This step confirms that the sample was properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and includes additional data entry if not available through electronic custody systems.

Screening acts as an initial check for illegal substances. Though Screening presents an economical method to eliminate substance use in most samples, a positive result must be confirmed for legal admissibility. Presumptive positives in Screening necessitate secondary verification.

When a sample appears positive during Screening, additional hair is extracted from the collected sample for Extraction. In this phase, drugs are removed from hair at significantly lower levels than in other methods (e.g., urine or oral fluid), which makes hair drug testing the most challenging process to execute.

Confirmation for any preliminary positive result occurs via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation if necessary. The entire lab process from Accessioning to Confirmation is inspected under the CAP (College of American Pathologists) Hair designation and meets the accreditation to ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
