What it is, How it works

When obtained near the scalp, hair offers up to an approximate 3-month detection span for alcohol and drugs. It is easy to collect, somewhat resistant to tampering, and simple to transport.

A sample measuring 1.5 inches from about 200 hair strands (roughly the size of a #2 pencil) closest to the scalp will yield 100mg of hair, which is optimal for screening and confirmation. For EtG, additional tests, or tests exceeding 10 panels, a sample of 150mg is suggested. We advise weighing the sample using a jeweler’s scale. If scalp hair is unavailable, a corresponding amount of body hair may be collected. Our reference to head hair pertains solely to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory process of a drug test result encompasses four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning deals with the initial incorporation of a sample into a lab's system. This process includes ensuring the sample was sealed and transported correctly, assigning an arbitrary LAN (Laboratory Accessioning Number), and completing any further data input not offered by an electronic chain of custody system.

Screening serves as an initial rapid assessment for the presence of abusive drugs. While Screening is a cost-efficient approach for eliminating drug usage in most samples, a positive screening result requires confirmation to be admissible in court. Any samples that show a presumptive positive in Screening necessitate a follow-up confirmation.

When a sample shows a presumptive positive in the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. At this stage, drugs are derived from hair at significantly lower concentrations than other methodologies (e.g., urine or oral fluid), which underscores why hair drug analysis is the most challenging methodology to conduct.

Confirmation of any positive screening outcome is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation as required. The entire lab process, from Accessioning to Confirmation, is examined under both the CAP (College of American Pathologists) Hair designation and the accreditation in accordance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
