When obtained near the scalp, hair can reveal drug and alcohol use over an approximate 3-month period. Hair collection is straightforward, somewhat resistant to tampering, and convenient to transport.
A sample measuring 1.5 inches in length, consisting of roughly 200 hair strands (similar in size to a #2 pencil), nearest to the scalp will yield 100mg, which is the optimal amount for both screening and confirmation. For EtG and additional tests exceeding 10 panels, 150mg is advisable. We suggest using a jeweler’s scale to weigh the sample. If scalp hair isn’t available, a similar amount of body hair can be used. When mentioning head hair, we are only referring to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).
Process Overview
The laboratory processing of a drug test consists of four major phases: Accessioning, Screening, Extraction, and Confirmation.
Accessioning is the initial phase where a sample is entered into the lab's system. This phase ensures that the sample was sealed and transported appropriately, assigns a random LAN (Laboratory Accessioning Number), and completes any necessary documentation not included in an electronic chain of custody.
Screening is a preliminary assessment for substances of abuse. Although it is a cost-effective method to eliminate drug usage in most samples, any positive results from Screening must undergo confirmation to be valid in legal proceedings. Any samples testing positive in Screening require further confirmation.
Should a sample test positive in the Screening stage, more hair is extracted from the original specimen and prepared for Extraction. In this stage, substances are extracted from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), which makes hair drug testing particularly challenging.
Confirmation of a positive Screening outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. Samples presumed positive are washed before confirmation, if needed. The comprehensive lab process, from Accessioning to Confirmation, is conducted under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.
The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
