A 1.5-inch sample comprising approximately 200 hair strands (equivalent to the thickness of a #2 pencil) near the scalp will yield 100mg, considered the optimal sample for both screening and subsequent confirmation. For EtG, additional testing, and/or analysis exceeding 10-panel tests, 150mg of hair is advisable. It's suggested to use a jeweler's scale for weighing. Should scalp hair be inaccessible, an equivalent volume of body hair can be used. By head hair, we refer exclusively to scalp hair, whereas body hair includes all other types (e.g., facial, axillary).

Process Overview

The drug testing laboratory process is composed of four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample into a lab's workflow. This step involves confirming correct sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry beyond what is provided by an electronic chain of custody system.

Screening entails an initial, rapid evaluation for substance abuse. While this method is economically efficient for excluding substance consumption in most samples, legal admissibility requires confirming any positive findings. Samples showing presumptive positive results during Screening necessitate a subsequent confirmation.

In the case of a presumptively positive result during Screening, additional hair is extracted from the existing sample for Extraction. At this stage, substances are extracted from hair at significantly lower concentrations compared to other methods (such as urine or oral fluid), underscoring the complexity of hair drug screening.

Confirmation of any positive screening outcome is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positive samples undergo washing as necessary. From Accessioning to Confirmation, the entire lab process is scrutinized under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
