For EtG, add-ons, and tests exceeding 10 panels, 150mg of the sample is advised. Using a jeweler’s scale is recommended for accurate weighing of the sample. If scalp hair isn't available, one may collect an equivalent amount of body hair. Note that when referring to head hair, it specifically means scalp hair. Body hair includes all other types such as facial and underarm hair.

Process Overview

There are four key stages in laboratory processing for a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is where a sample is initially entered into a lab’s system. This stage ensures the sample was properly sealed and sent, assigns a random LAN (Laboratory Accessioning Number), and completes any remaining data entry not handled by an electronic chain of custody system.

Screening involves an initial, quick drug abuse check. While Screening is a cost-effective method to rule out drug usage in most samples, positive results need confirmation to be court-admissible. Any suspected positive samples from Screening require a secondary confirmation.

If a sample is suspected positive during Screening, further hair is taken from the initial sample and prepared for Extraction. Drugs are extracted from the hair at much lower concentrations than in other methods (e.g., urine or oral fluid), which is why hair drug testing is considered the most challenging methodology.

Confirmation of any positive screening results is done using GC/MS, GC/MS/MS, or LC/MS/MS. All suspected positive samples undergo washing prior to any necessary confirmation. The entire lab process, from Accessioning to Confirmation, is subject to review under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
