For EtG enhancements and/or screenings exceeding 10 panels, a specimen of 150mg is advisable. It is suggested to use a jeweler’s scale for measuring the sample weight. In cases where scalp hair is inaccessible, an equivalent quantity of body hair may be used. The term "head hair" refers solely to scalp hair, while "body hair" includes all other types such as facial and axillary.

Process Overview

The laboratory processing of drug test results includes four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning signifies the preliminary stage where a specimen is logged into the laboratory’s system. This phase involves confirming that the sample is adequately sealed and dispatched, assigning a unique LAN (Laboratory Accessioning Number), and completing any outstanding data input not covered by an electronic chain of custody.

Screening serves as an initial, swift evaluation for the presence of drugs. While Screening is an economical approach for ruling out drug usage in the majority of cases, positive screens must be verified to ensure their validity in legal contexts. Samples that are preliminarily positive during Screening require subsequent confirmation.

If the Screening phase yields a presumptive positive result, further hair is extracted from the existing specimen and prepped for Extraction. In this phase, drug substances are obtained from hair at significantly lower concentrations compared to other processes such as urine or oral fluid testing, rendering hair drug screening one of the most challenging procedures to execute.

The Confirmation of any positive result from Screening involves methods such as GC/MS, GC/MS/MS, or LC/MS/MS. All samples flagged as presumptive positives undergo cleaning before confirmation when necessary. The full laboratory procedure, from Accessioning to Confirmation, is inspected under the CAP (College of American Pathologists) Hair guidelines and in accordance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
