For EtG, additional tests, and scenarios exceeding 10 panels, 150mg of the sample is advised. It is advisable to weigh the sample using a jeweler’s scale. If scalp hair isn’t available, an equivalent quantity of body hair may be gathered. In reference to head hair, it specifically means scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four essential phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary handling of a sample within a laboratory’s framework. This step ensures the sample was properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and finalizes any supplementary data entry absent from electronic chain of custody records.

This process withdraws drugs from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), presenting hair drug screening as one of the most complex methodologies.

Confirmation of any positive screening outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo necessary washing before confirmation. The entire lab process, from Accessioning to Confirmation, is evaluated following both the CAP (College of American Pathologists) Hair criteria and certification to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
