A sample of 1.5 inches, consisting of roughly 200 hair strands (similar to the girth of a #2 pencil) from the scalp, yields 100mg, which is the optimum quantity for testing. For EtG, additional tests, and assessments beyond 10 panels, a quantity of 150mg is advised. We suggest using a jeweler’s scale for precision. Should scalp hair not be accessible, equivalent body hair may suffice. References to head hair denote only scalp hair. Body hair comprises all other types (facial, axillary, etc.).

Process Summary

The laboratory drug testing workflow encompasses four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves introducing the sample into the lab’s system, confirming proper sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and inputting any additional data absent from an electronic chain of custody system.

Screening performs an initial examination for drug traces. Though cost-efficient for eliminating drug presence in most samples, confirmed results are requisite for legal admission. Positive screening necessitates additional confirmation.

Upon a presumptive positive in Screening, extra hair is taken from the initial sample and prepared for Extraction. At this stage, drugs are removed from hair at lower concentrations compared to other methods (e.g., urine or saliva), underlying the complexity of hair drug testing.

Confirmation follows any positive screen via GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positives are washed as required. The entire process, from Accessioning to Confirmation, is scrutinized under CAP (College of American Pathologists) Hair criteria and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
