A 1.5-inch portion of approximately 200 hair strands (around the size of a #2 pencil) closest to the scalp yields 100mg, which is the optimal amount for screening and confirmation. For EtG, additional tests, and/or those exceeding 10 panels, collecting 150mg is advised. We suggest using a jeweler’s scale to measure the specimen. If scalp hair is not accessible, an equivalent amount of body hair can be used. Scalp hair refers specifically to head hair, while body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves the four main steps of Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of entering a sample into a lab's system. It verifies sample sealing and proper shipment, assigns a random LAN (Laboratory Accessioning Number), and concludes any required data entry missing from an electronic chain of custody system.

Screening provides a primary check for the presence of drugs. Though cost-effective in excluding drug use in most samples, a positive screen must be validated for judicial admissibility. Any samples preliminarily positive in Screening require further confirmation.

Samples presumptively positive during Screening undergo Extraction, where more hair is drawn from the initial sample. Here, drugs are extracted at lower concentrations than other methods (i.e., urine or oral fluid), making hair drug screening among the most intricate methodologies.

Positive screening results are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are washed before confirmation if necessary. The entire process from Accessioning to Confirmation is conducted under the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.
