What it is, How it works

A 1.5-inch segment of approximately 200 hair strands (comparable to a #2 pencil) nearest to the scalp will provide 100mg of hair, the optimal quantity for both screening and confirmation. For EtG, add-ons, and/or analyses exceeding 10 panels, a 150mg sample is advised. It's recommended to measure the sample with a jeweler’s scale. If scalp hair is unavailable, an equivalent amount of body hair can be used. When discussing head hair, we refer exclusively to scalp hair. Body hair includes all other types of hair (facial, axillary, etc.).

Process Overview

There are four primary stages in processing a drug test in the lab: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial sample processing into a lab's system. It involves ensuring the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any further data entry not covered by an electronic chain of custody system.

Screening provides an initial assessment for drugs. Screening is cost-effective for excluding drug use in most samples, but a positive screening result requires confirmation for legal acceptability. Presumptively positive samples in Screening necessitate further confirmation.

For samples that test presumptively positive at the Screening stage, more hair is extracted from the original sample in preparation for Extraction. Here, drugs are derived from the hair at lower concentrations compared to other methods (like urine or oral fluid), highlighting the challenges of hair drug screening.

Confirmation of a positive screen is done using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that test presumptive positive undergo washing prior to confirmation if necessary. The complete lab process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
