For EtG, additional testing, and/or panels exceeding ten, 150mg of the specimen is advisable. Using a jeweler’s scale for weighing the specimen is recommended. In the absence of scalp hair, an equivalent quantity of body hair can be utilized. When mentioning head hair, it specifically refers to scalp hair only. All other hair types (e.g., facial, axillary, etc.) are classified as body hair.

Process Overview

The key stages in the lab processing of drug test results include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the primary stage where a sample is entered into the lab’s system. It involves ensuring that the sample was suitably sealed and transported, assigning a unique LAN (Laboratory Accessioning Number), and completing any data entry not covered by an electronic chain of custody system.

Screening involves a preliminary rapid inspection for drugs of abuse. Although Screening is a budget-friendly approach for eliminating drug usage in most cases, a positive screen requires confirmation for court acceptance. Samples that test positive initially in Screening need a confirmatory test.

In cases where a sample is initially positive during Screening, additional hair is extracted from the original specimen to prepare for Extraction. In this phase, drugs are extracted from the hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening exceptionally challenging.

Affirmation of any positive Screening outcome is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positively screened samples are cleansed before confirmation if necessary. The lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
