Utilizing hair testing is now regarded as a robust method for identifying the usage of drugs and alcohol. Hair acts as a reliable timeline of alcohol and drug consumption by encapsulating biomarkers within the strands of hair as it grows. When samples are gathered from near the scalp, hair can offer up to an estimated 90-day window for detecting alcohol and drugs. The collection of hair is straightforward, somewhat resistant to tampering, and uncomplicated for shipping.
A sample measuring 1.5 inches, consisting of around 200 strands of hair (similar to a #2 pencil in thickness) nearest to the scalp, will yield 100mg of hair, which is the optimal sample size for testing and verification. For EtG, additional testing, and/or panels exceeding ten, 150mg of the specimen is advisable. Using a jeweler’s scale for weighing the specimen is recommended. In the absence of scalp hair, an equivalent quantity of body hair can be utilized. When mentioning head hair, it specifically refers to scalp hair only. All other hair types (e.g., facial, axillary, etc.) are classified as body hair.
Process Overview
The key stages in the lab processing of drug test results include Accessioning, Screening, Extraction, and Confirmation.
Accessioning is the primary stage where a sample is entered into the lab’s system. It involves ensuring that the sample was suitably sealed and transported, assigning a unique LAN (Laboratory Accessioning Number), and completing any data entry not covered by an electronic chain of custody system.
Screening involves a preliminary rapid inspection for drugs of abuse. Although Screening is a budget-friendly approach for eliminating drug usage in most cases, a positive screen requires confirmation for court acceptance. Samples that test positive initially in Screening need a confirmatory test.
In cases where a sample is initially positive during Screening, additional hair is extracted from the original specimen to prepare for Extraction. In this phase, drugs are extracted from the hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening exceptionally challenging.
Affirmation of any positive Screening outcome is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positively screened samples are cleansed before confirmation if necessary. The lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.
