What it is, How it works

Hair offers a comprehensive history of substance use by encapsulating biomarkers within the fibers of the hair as it grows. When taken close to the scalp, hair can reveal drug and alcohol use over a period of roughly three months. Hair is easy to collect, hard to tamper with, and simple to transport.

A sample of 1.5 inches, approximately 200 strands of hair (similar in size to a #2 pencil) nearest to the scalp equates to 100mg of hair, which is optimal for screenings and confirmations. For EtG, additional tests, or panels exceeding 10, a specimen of 150mg is advised. Using a jeweler’s scale for weighing the specimen is recommended. If scalp hair is not available, an equivalent amount of body hair can be used. When mentioning head hair, we exclusively mean scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of drug tests encompasses four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning begins with enrolling a sample into a lab's framework. This step ensures the sample was properly sealed and delivered, assigns a unique LAN (Laboratory Accessioning Number), and completes any supplementary data entry absent from an electronic chain of custody.

Screening is a swift preliminary examination for the presence of drugs. Although Screening is an efficient way to eliminate drug use for the majority of samples, any positive screening requires confirmation for legal admissibility. Samples presumptively positive during Screening necessitate further confirmation.

Should a sample be deemed presumptively positive in Screening, additional hair is sourced from the original specimen and processed for Extraction. In Extraction, drugs are isolated from hair at much lower concentrations than in other mediums (e.g., urine or oral fluids), explaining why hair drug tests are challenging to perform.

Confirmation of any presumptive positive result is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. All samples indicated as presumptive positives are cleansed before confirmation as needed. The laboratory procedures from Accessioning to Confirmation adhere to the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
