A 1.5-inch segment consisting of around 200 hair strands (comparable to the thickness of a #2 pencil) nearest to the scalp will produce 100mg, which is the ideal amount for both testing and confirmation. For tests involving EtG, additional components, and/or over 10 panels, a 150mg sample is advisable. We suggest weighing the sample using a jeweler’s scale. Should scalp hair be unavailable, a corresponding quantity of body hair can be used. References to head hair specifically mean scalp hair. Body hair encompasses all other hair types (such as facial and axillary hairs).

Process Overview

The laboratory procedure for processing a drug test outcome consists of four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary handling of a specimen into a laboratory's framework. This task includes ensuring that the sample was appropriately sealed and dispatched, assigning it a random LAN (Laboratory Accessioning Number), and finishing any supplementary data entry missing from an electronic chain of custody system.

Screening performs a swift initial evaluation for drugs. While Screening is a cost-effective strategy to disqualify drug use in most samples, a positive result requires further confirmation for legal proceedings. Any samples with a presumptive positive from Screening need a further confirmation step.

If Screening suggests a positive result, additional hair from the original sample is utilized for the Extraction process. Here, drugs are isolated from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), which makes hair drug testing one of the more complex methodologies.

Confirmation for any presumptively positive results from Screening is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive specimens are cleansed before confirmation, if necessary. From Accessioning to Confirmation, the comprehensive laboratory process is scrutinized under both the CAP (College of American Pathologists) Hair criteria and accreditation according to ISO / IEC 17025 norms.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
