What it is, How it works

A 1.5-inch sample of around 200 hair strands (similar in size to a #2 pencil) near the scalp yields 100mg, which is the perfect sample size for screening and confirmation. For EtG, additional, and/or tests over 10 panels, a specimen of 150mg is advised. Using a jeweler’s scale to weigh the sample is suggested. If scalp hair is not available, the same amount of body hair can be taken. When discussing head hair, it refers specifically to scalp hair. Body hair covers all other types of hair (facial, axillary, etc.).

Process Overview

The four primary stages in laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning marks the initial input of a sample into the lab's system. This step checks that the sample is sealed and sent correctly, assigns an LAN (Laboratory Accessioning Number), and completes any extra data entry missing from an electronic chain of custody system.

Screening is a preliminary check for drug presence. It is a cost-effective approach to rule out drug usage in most samples. However, positive screens must be confirmed for court admissibility, and samples that are presumptively positive in Screening necessitate further confirmation.

For samples presumed positive in Screening, additional hair is taken from the original sample and prepared for Extraction. During this phase, drugs are extracted from hair at lower concentrations than other methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to perform.

Confirmation of any positive results from screening is done using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleaned before confirmation as required. The whole process from Accessioning to Confirmation undergoes review under the CAP (College of American Pathologists) Hair standard and ISO / IEC 17025 accreditation.
