What it is, How it works

Hair testing is recognized as an effective technique for identifying drug and alcohol use. Hair offers a prolonged record of substances by capturing biomarkers within the fibers of the growing strand. When obtained near the scalp, hair can present a detection period of up to about 3 months for alcohol and other drugs. Hair collection is straightforward, relatively resistant to tampering, and easy to transport.

A sample measuring 1.5 inches, composed of approximately 200 hair strands (around the thickness of a #2 pencil) nearest to the scalp, will yield 100mg, which is ideal for screening and confirmation. For EtG, add-ons, and tests beyond 10 panels, 150mg of the sample is suggested. We advise using a jeweler’s scale for weighing. In the absence of scalp hair, an equivalent quantity of body hair may be gathered. When mentioning head hair, it pertains solely to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The four primary stages in the lab analysis of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial integration of a sample into a laboratory’s system. This step ensures the sample was sealed and shipped correctly, assigns a random LAN (Laboratory Accessioning Number), and finalizes any supplementary data entry not covered by an electronic chain of custody system.

Screening is a preliminary assessment for drugs of abuse. While Screening is a cost-effective method to eliminate drug usage in most samples, a positive screen must be confirmed to be admissible in legal settings. Any samples showing presumptive positivity in Screening require further confirmation.

If a sample yields a presumptive positive in Screening, additional hair is withdrawn from the initial sample and set for Extraction. During this phase, drugs are isolated from hair at a significantly lower concentration compared to other methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology.

Confirmation of any positive screening outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. Any samples showing presumptive positivity are cleansed before confirmation if necessary. The total lab procedure from Accessioning to Confirmation is assessed under the CAP (College of American Pathologists) Hair designation and conforms to the accreditation standards of ISO / IEC 17025.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
