Hair analysis has gained recognition as a potent method for identifying drug and alcohol consumption. Hair offers an extended timeline of alcohol and drug use by encapsulating biomarkers within the structure of the growing hair strand. When gathered near the scalp, hair can reveal approximately a 3-month timeline for alcohol and drug detection. Collecting hair is straightforward, challenging to tamper with, and easy to transport.
A 1.5-inch segment consisting of roughly 200 hair strands (approximately the width of a #2 pencil) closest to the scalp provides 100mg of hair, which is the optimal sample size for screening and confirmation. For EtG, additional panels, and tests exceeding 10 panels, 150mg of the sample is suggested. Using a jeweler’s scale to weigh the specimen is advised. If scalp hair is unavailable, a comparable amount of body hair may be used. By head hair, we mean scalp hair only. Body hair encompasses all other hair types (facial, axillary, etc.).
Process Overview
The primary stages in the laboratory analysis of a drug test result include Accessioning, Screening, Extraction, and Confirmation.
Accessioning entails the preliminary processing of a sample into the laboratory’s system. This procedure involves confirming the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and performing any supplementary data entry not included by an electronic chain of custody system.
Screening is a preliminary rapid detection of drugs. Although Screening is a cost-efficient method to exclude drug usage in most samples, a positive screen must be confirmed for court admissibility. Samples that are presumptively positive in Screening require further confirmation.
In the event a sample is presumptively positive during Screening, additional hair is extracted from the initial specimen and processed for Extraction. During this phase, drugs are extracted from hair at much lower concentrations than in other methods (e.g., urine or oral fluid), which makes hair drug screening the most challenging methodology.
Confirmation of any positive screening result is conducted through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples undergo washing prior to confirmation as necessary. The complete laboratory process from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
