A sample measuring 1.5 inches, consisting of about 200 hairs (similar to the diameter of a #2 pencil) near the scalp, provides 100mg of hair, which is an optimal quantity for screening and confirmation. For analyses like EtG, add-ons, or tests exceeding 10 panels, a specimen weighing 150mg is advised. It's suggested to use a jeweler’s scale for weighing the specimen. If scalp hair is absent, an equivalent quantity of body hair can be used. The term head hair specifically refers to scalp hair, whereas body hair encompasses all other types (like facial or axillary).

Process Overview

There are four principal steps in laboratory drug test processing: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary step where a sample is integrated into the laboratory's system. This involves ensuring the sample's seal and shipment integrity, assigning it a unique LAN (Laboratory Accessioning Number), and inputting any additional data not covered by an electronic chain of custody.

Screening is a preliminary inspection for drugs of abuse. While Screening offers an economical approach for negating drug use in most samples, a positive result requires confirmation for legal acceptability. Samples with presumptive positive results in Screening necessitate secondary confirmation.

When a Screening step results in a presumptive positive, more hair is utilized from the original specimen for the Extraction phase. At this stage, drugs are extracted from hair at a substantially lower concentration compared to other methods (like urine or oral fluid), which makes hair drug screening the most challenging to conduct.

Confirmation for any positive screenings is executed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Before confirmation, all presumptive positive samples undergo washing as required. The comprehensive laboratory process from Accessioning to Confirmation is scrutinized following the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
