Where scalp hair is not present, an equivalent measure of body hair can be used. Head hair references only scalp hair, whereas body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the primary processing of a sample within a laboratory system. It involves ensuring the sample is properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing supplementary data entries absent from an electronic chain of custody system.

Screening entails a preliminary, rapid evaluation for drug abuse. Although cost-effective for negating drug usage in most samples, a positive screen mandates confirmation to be legally valid. Samples with presumptive positivity in Screening require secondary validation.

For specimens showing potential positivity during Screening, additional hair is extracted from the initial sample for the Extraction process. This phase involves isolating drugs from hair at lower concentrations than in other methods (e.g., urine or oral fluid), rendering hair drug screening the most complex methodology.

Positive screening outcomes are confirmed through techniques like GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation as necessary. From Accessioning to Confirmation, the full laboratory process adheres to CAP (College of American Pathologists) Hair designation and is accredited to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
