What it is, How it works

Hair analysis has emerged as a significant method for identifying drug and alcohol consumption. When clipped near the scalp, hair can offer an approximate 3-month timeframe for detecting alcohol and other substances. Hair is straightforward to gather, relatively resistant to tampering, and simple to transport.

A 1.5-inch sample comprising around 200 hair strands (roughly similar to the diameter of a #2 pencil) cut closest to the scalp will produce 100mg, which is the optimal amount for both screening and confirmation. For EtG, additional analyses, and/or tests exceeding 10 panels, 150mg of the specimen is advised. We suggest using a jeweler’s scale for accurate specimen weighing. If scalp hair is not accessible, a comparable quantity of body hair can be obtained. References to head hair specifically mean scalp hair only. Body hair encompasses all other types, including facial and underarm hair.

Process Overview

The laboratory workflow for processing drug test results involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial steps of entering a sample into a laboratory's database. This step includes confirming the sample was properly sealed and transported, assigning a unique LAN (Laboratory Accessioning Number), and completing any necessary data entry tasks not covered by an electronic custody system.

Screening conducts a preliminary assessment for substance abuse. While being a cost-efficient solution to exclude drug usage in the majority of samples, a positive screening result must be verified for legal validity. Any samples showing preliminary positive results during Screening require an additional confirmation process.

Upon a presumptive positive result in the Screening phase, additional hair is drawn from the original sample and prepped for Extraction. In this stage, drugs are isolated from hair in considerably lower concentrations than in alternative methods (like urine or oral fluids), making hair drug testing notably challenging compared to other practices.

Confirmation of all positive screening findings is undertaken using GC/MS, GC/MS/MS, or LC/MS/MS. All samples showing presumptive positives undergo washing before confirmation as necessary. The entirety of the laboratory operations from Accessioning through Confirmation is assessed under the guidelines of the CAP (College of American Pathologists) Hair classification and in compliance with ISO / IEC 17025 standards.
