When hair is collected near the scalp, it offers a potential 3-month detection window for alcohol and other substances. Collecting hair is straightforward, hard to tamper with, and easy to transport.

A 1.5-inch sample with about 200 strands of hair (approximately the width of a #2 pencil) nearest the scalp equals 100mg, the preferred quantity for screening and verification. For EtG, additional tests, or tests exceeding 10 panels, 150mg is advised. We suggest weighing the hair on a jeweler’s scale. If scalp hair is absent, a similar quantity of body hair can be used. When noting head hair, we refer solely to scalp hair. Body hair includes all other forms of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the preliminary handling of a sample in the lab's system. This step verifies sample sealing and shipment accuracy, assigns a random LAN (Laboratory Accessioning Number), and completes any other required data entry not facilitated by an electronic chain of custody system.

Screening entails a preliminary swift assessment for drugs of abuse. Screening offers a cost-efficient means to eliminate drug use in most samples, but a positive screen must be verified to be court-admissible. Presumptively positive screening samples require subsequent confirmation.

Should a sample be presumptively positive during Screening, additional hair from the initial sample is readied for Extraction. Here, drugs are extracted from hair at a lower concentration compared to other techniques (e.g., urine or oral fluid), rendering hair drug screening a challenging methodology to execute.

Any positive screening outcomes undergo Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are cleansed prior to confirmation if necessary. The complete process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
