Hair testing has gained recognition as an effective tool for identifying drug and alcohol consumption. Hair offers a long-term record of alcohol and drug use by embedding biomarkers within the fibers of the growing hair strand. When taken close to the scalp, hair can provide a detection window of up to roughly 3 months for alcohol and other drugs. Collecting hair is straightforward, relatively resistant to tampering, and simple to transport.
A 1.5-inch sample of roughly 200 hair strands (about the diameter of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal amount for screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, a 150mg sample is advised. It is suggested to weigh the specimen using a jeweler’s scale. If scalp hair is not available, the same quantity of body hair may be collected. When mentioning head hair, we specifically mean scalp hair. Body hair includes all other kinds (facial, axillary, and so on).
Process Overview
The key steps in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.
Accessioning involves initially processing a sample into a laboratory’s system. This step includes verifying the sample was properly sealed and shipped, assigning a unique LAN (Laboratory Accessioning Number), and completing any further data entry not covered by an electronic chain of custody system.
Screening involves a preliminary rapid analysis for drugs of abuse. Although Screening is a cost-effective method to rule out drug usage on most samples, a positive screen must be confirmed to be valid in court. Any samples that appear positive in Screening need further confirmation.
If a sample tests presumptively positive in the Screening stage, more hair is extracted from the initial specimen for further Extraction. During this phase, drugs are extracted from hair at much lower concentrations compared to other methods (ex. urine or oral fluid), which makes hair drug screening the most challenging methodology to execute.
Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before confirmation if required. The complete lab process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
