A 1.5-inch segment of approximately 200 hair strands (comparable to a #2 pencil width) near the scalp yields 100mg of hair, ideal for screening and confirmation. For EtG, add-ons, and/or tests beyond 10 panels, 150mg is advised. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair can't be obtained, a similar quantity of body hair may be used. When mentioning head hair, it refers solely to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The principal stages in the lab processing of a drug test include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial processing phase, where a sample is entered into the lab’s system. This step ensures the sample was sealed and transported correctly, a random LAN (Laboratory Accessioning Number) is assigned, and any additional data entry not provided by an electronic chain of custody system is completed.

Screening provides an initial rapid assessment for drug use. Screening is a cost-effective means to eliminate drug presence in most samples; however, a positive screen must be verified to be admissible in legal settings. Samples that screen positive require a following confirmation step.

For samples with a presumptive positive result during Screening, more hair is taken from the initial sample and readied for Extraction. In Extraction, drugs are separated from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug tests the most challenging method to execute.

GC/MS, GC/MS/MS or LC/MS/MS is used for Confirmation of any positive screenings. Prior to confirmation, all samples with a presumptive positive result are washed as necessary. The entire process, from Accessioning to Confirmation, is evaluated in compliance with both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
