What it is, How it works

Hair assessments have emerged as an effective means for identifying drug and alcohol consumption. Hair captures a prolonged history of substance use by embedding biomarkers within the fibers of developing hair strands. When gathered near the scalp, hair can offer a detection timeframe of up to roughly 3 months for both alcohol and other substances. Hair is straightforward to compile, relatively resistant to tampering, and easy to transport.

A sample measuring 1.5 inches consisting of around 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp provides 100mg of hair, which is the optimal amount for initial tests and subsequent verifications. For EtG, additional evaluations, and/or tests exceeding 10 panels, a 150mg sample is advised. We suggest using a jeweler's scale for precise specimen measurement. If no scalp hair is present, a similar quantity of body hair may be used. When mentioning head hair, we specifically mean scalp hair. Body hair pertains to all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result includes four major steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed into a lab’s infrastructure. This step involves confirming that the sample was appropriately sealed and dispatched, attributing a random LAN (Laboratory Accessioning Number), and completing any additional data input absent from an electronic chain of custody system.

Screening serves as a preliminary assessment for the presence of substances. While Screening is efficient in terms of cost for eliminating drug use in most samples, any positive findings must be verified to be acceptable in legal proceedings. Samples showing presumptive positivity in Screening mandate further confirmation.

If a sample indicates presumptive positivity during Screening, additional hair from the original specimen is designated for Extraction. Here, drugs are removed from hair in significantly reduced concentrations compared to other techniques (e.g., urine or oral fluid), highlighting the complexity of hair drug screening.

Verification of any positive Screening outcomes is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples indicating presumptive positivity are cleaned before verification as necessary. The complete lab process from Accessioning through Confirmation is checked according to the CAP (College of American Pathologists) Hair criteria and the ISO / IEC 17025 accreditation standards.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
