A 1.5-inch section with roughly 200 hair strands (comparable in size to a #2 pencil) located nearest the scalp provides 100mg of hair, which is an ideal amount for both screening and confirmation. For EtG, additional tests, and/or if the panel exceeds 10, 150mg is advisable. A jeweler's scale is recommended for weighing the specimen. In cases where scalp hair isn't available, an equivalent volume of body hair may be used. References to head hair pertain only to scalp hair, whereas body hair includes all other types (such as facial or underarm hair).

Process Overview

There are four principal stages in processing a drug test result in the lab: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial processing of a sample in a lab's system. This step ensures the sample was appropriately sealed and sent, assigns a unique LAN (Laboratory Accessioning Number), and fills in any missing data not captured by an electronic chain of custody.

Screening involves a preliminary quick assessment for substance abuse. Though Screening is a cost-efficient method to mostly rule out drug use, a positive result must be verified to be valid in court. Samples with a presumptive positive result in Screening require further confirmation.

Should a sample show presumptive positivity in the Screening phase, more hair is drawn from the original specimen and readied for Extraction. Here, drugs are extracted from hair at concentrations significantly lower than in other methods (such as urine or oral fluid), underscoring the complexity of hair drug analysis.

Verification of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives undergo washing before confirmation as necessary. The entire lab procedure from Accessioning to Confirmation is assessed with both the CAP (College of American Pathologists) Hair designation and accreditation per ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
