For EtG, additional testing, and/or exams beyond 10 panels, a 150mg specimen is optimal. Using a jeweler’s scale to weigh the specimen is advised. In cases where scalp hair is not accessible, a comparable portion of body hair may be used. By head hair, we mean scalp hair solely. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory analysis of a drug test comprises four essential phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the preliminary handling of a sample into the laboratory’s database. This phase includes confirming the sample’s proper sealing and shipping, assigning a unique LAN (Laboratory Accessioning Number), and completing any unfulfilled data entry absent from an electronic chain of custody system.

Screening serves as an initial rapid assessment for drug misuse. While cost-effective for excluding drug usage in most samples, a positive screen demands confirmation to stand up in court. Samples indicating presumptive positivity in Screening require subsequent confirmation.

Upon a presumptive positive result in the Screening phase, additional hair is extracted from the original specimen for Extraction. Drugs are then isolated from hair at lower concentrations compared to other methods (e.g., urine or oral fluid), marking hair drug screening as the most challenging methodology.

Positive screens undergo confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. Before Confirmation, all presumptive positive samples are cleansed if required. The complete laboratory procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
